Faulty driver.
Once upon a time we had a DNS server that would lose data. It would transfer that loss to the backup server. I'd get a call in the dead of the night.
It WAS a printer driver. Have to love Ubersoft at times.
Cost an arm and a leg
In one of the panels you can see the character's leg and arm beneath the panel below it.
